Transcriber’s Notes: There are many instances of words and names that are hyphenated in the body of the book, but not in the index. There are a few that are hyphenated in the index, but not in the body of the book. The hyphenation was left as printed. Missing or obscured punctuation was silently corrected. Typographical errors were silently corrected. Inconsistent spelling and hyphenation were made consistent only when a predominant form was found in this book. The footnotes have been gathered into one section at the end of the book.
